Parent or Legal Guardian,

You are required to read the following information very carefully and make sure that you understand it fully before allowing your child to participate in this activity or program.

I am fully aware that participation in the Teen Summer Program may result in risk of personal injury or harm including but not limited to: accidents related to running, playing outdoors, and playing in water to my child.

I hereby agree to release and hold harmless the West Bonner Library District its officers, employees, volunteers, committees and boards, from and against any and all liability, loss, damages, claims, or actions (including costs and attorney’s fees) for bodily injury and/or property damage, to the extent permissible by law.

This indemnification and hold harmless agreement shall include indemnity against all costs (including without limitation, reasonable attorney's fees and court costs), expenses and liabilities incurred in or in connection with any such claim or proceeding brought thereon and in defense thereof.

Your child may be photographed or videotaped for library publicity purposes. We may use these for reports, presentations and social media. Please alert library staff before an event if you do not want your child’s photo taken. No compensation of any kind will be paid for the use of your child’s likeness.

In signing this release, indemnification and hold harmless form, I acknowledge that I have read and understand fully the foregoing agreement and sign it voluntarily as my own free act and deed; no oral representations, statements, or inducements, apart from the foregoing written agreement, have been made. I hereby give permission to the West Bonner Library District for emergency transportation and/or treatment by medical professionals in the event of illness or injury. I hereby accept responsibility for the payment of any emergency transportation and/or treatment. I further certify that I am fully competent, and my child is in good physical condition, and has no medical or physical conditions that would restrict my child's participation in any program or activity.

I understand that if I choose to leave my child at the Teen Summer Program I must be back by 12:45pm to pick my child up. The library staff and volunteers do not check ID at pick up. In addition, library staff and volunteers will not apply first aid directly, such as putting on a band-aid or an ice pack. Completion of this registration constitutes agreement to this waiver.
