Connections Area Agency on Aging 2026-2029 Area Plan on Aging.  All information provided is confidential. This survey is designed to identify community strengths, needs and challenges for the 60+ population, persons living with disabilities, and informal caregivers. Group responses will be used to create the 4-year plan for aging services in our community. The survey is divided into four (4) sections: Demographics, Community Independent Living Needs, Individual Independent Living Needs, and Family Caregiving. The following survey will provide valuable information and help shape the services and supports  provided to older adults, persons living with disabilities, and informal caregivers living and with your community for years to come.
