Safety Concern Form
Hello AVID <3
This form is for you to let us know if there are any safety concerns (defined as any interpersonal conflict causing you to feel uncomfortable, unsafe, disoriented) when attending any of our events. Someone will check this weekly on Mondays at 5 PM/MST.

This is an anonymous form. There will be an option at the end if you want to leave your name and email address so we can check in with you or get clarification about your concern. We ask you to describe the incident in details so we have a better understanding of what happened and how we (AVID organizers) can address it. 

Our next steps as AVID organizers is to address your concern with the individual(s) involved with dignity and respect to create a safer, culture of care in our community. Thank you for your time and care!
