Please complete and submit this Application for Student Enrollment using the form below. This is the first step to a complete application. The Student Snapshot is at http://lakelandmontessori.com/admissions/forms/middle-school-snapshot-forms and must be received for the application to be considered complete. Submitting this application will begin your enrollment process.
If you are completing applications for more than one of your children, there is no guarantee that all siblings will be able to attend the school, although they will all receive preference.
Enrollments are established by grade level and physical space.
Students that are retained forfeit their seat and must reapply for a new seat.
Exceptional student education students are subject to an IEP review prior to enrollment, falsification of information regarding level of required services will be grounds for referral to the appropriate district school.
Lakeland Montessori Schools have a contract with the Polk County School district to pay to transport our students that live more than two miles from the school and less than five miles from the school. In order to be consistent with all families, we determine the 5 miles as the distance measured from the closest pedestrian entry point of the property where the student resides to the closest pedestrian entry point of the school building that the student attends. We use Mapquest or Google maps, whichever is shorter, to determine the pedestrian distance.
It is your responsibility to advise us if you have a change of email address and/or phone number.
If you accept enrollment to Lakeland Montessori Schoolhouse, your child will NOT be taken off any other schools' waiting lists.
Thank you for your interest in Lakeland Montessori Middle School. If you have any questions, please email the Principal, Heather Manrow, at firstname.lastname@example.org.
With this in mind, LMMS expects that its students will• treat themselves, each other, the classroom, Polk Museum of Art, its grounds, and all staff with respect;• abide by the community standards for conflict resolution;• engage in productive and challenging work; and• participate in Service-‐Learning and Intersessions.
LMMS believes that parents are an important part of their child’s education. We expect that parents will
• volunteer at least 20 hours per family per school year;• require and encourage your child to participate in all Intersessions and Service-Learning trips;• be in agreement with LMMS’s mission and vision;• encourage their child’s peaceable interaction at home;• respect the community standards for conflict resolution; and• be committed to peaceable interaction with other parents, other students, and all staff.
LMMS staff are as unique as all the other aspects of the school. We expect that the staff will• be flexible, aware and available to students, parents, and other staff;• observe, guide and facilitate student education through “following the child”;• collaborate with staff and “specialists” as needed;• abide by community standards for conflict resolution; and• maintain a safe and prepared environment.
We are agreeing to abide by the terms listed above and to do our best to encourage all members of the community to uphold these standards. We agree that this community agreement is important to the LMMS community, and we are committed to all of the details listed above.
