Field Trip Order Form
Important Information

- Lunch orders must be received at least 10 days prior to the field trip
  
- Note: Meal items are subject to change based on availability.
  Orders that are placed less than 10 days prior, or changed at the last minute are not guaranteed to be your choice of meal. 

- A roster list OR lunch cards of the students taking the meals must be given to the cook at your school the day    PRIOR to the meals being handed off to you.

 - The following Schools will be picking up at North High:
   North, Cooper, Grant, Jefferson, Lincoln, Pigeon River, Etude Elementary, SLA, LCA, Urban

- The following schools will be picking up at South High:
  South, Jackson, Longfellow, Madison, Sheridan, ELC, Wilson, Horace Mann, Farnsworth

- The following schools will be picking up at Central:
  Central, GW HS/MS, CHANGE, Etude MS/HS

- The following schools will be picking up at Cleveland:
  Cleveland

- Grab 'n Go Style including: Whole Grains, Protein, Fruit, Veggies, Milk & Spork Kit
- Each meal meets the federal USDA National School Lunch Program dietary guidelines

- Please note that after a request is received, an email will be sent to the teacher(s), secretaries, and appropriate school nutrition professionals involved
