By submitting a nomination for the South Leicestershire School Sport & Well-being Awards, your nomination will be automatically pass on to the organisers of Sports Awards within your local authority (which occur later in the year).

Sports Team of the Year- A school team that has excelled in competitions, delivering excellent performances and results.
Sports Performer of the Year- A child or young person that has showcased outstanding performances as an individual and/or has made a significant difference to a team. An award based on excelling at sport.
Young Leader/ Leaders of the Year (Team as well as individual)- An individual or group of children/young people that have shown excellent leadership skills and have had a positive impact on school sport, physical activity and/or well-being. Examples could be Sports Ambassadors, Sports Leaders, Well-being Ambassadors and Playground Leaders.
Inspirational Child & Young Person Award-A child or young person that has inspired or helped others through school sports, physical activity and well-being. For example, being a strong role model or taking part in fundraising activities for a charity.
Staff Well-being Award- A school, individual or group that has showcased an outstanding commitment to supporting staff well-being.
School Well-Being Award- A school that has showcased an outstanding commitment to supporting well-being.
Sports Coach of the Year-A Sports Coach that has demonstrated an outstanding contribution to school sport.
Outstanding Contribution to School Sport and Well-being-A member of staff or volunteer that has showcased an outstanding commitment and contribution to school sport, physical activity and/or well-being over the academic year or several years.
Lifetime Achievement Award-An individual near or at the end of their career that has demonstrated outstanding commitment and contribution to school sport, physical activity and/or well-being over a number of years.
Miscellaneous-Any school, member of staff, volunteer, child or young person that is worthy of recognition. We would love to hear why they are worthy of recognition.
The data from this form will be stored on a spreadsheet incompliance with GDPR. The data from the form will be shared within the South Leicestershire School Sports Partnership. If your nominee is shortlisted, the South Leicestershire School Sport Partnership may share some of the information on this form. The South Leicestershire School Sports Partnership will continue to monitor how we manage this data and comply to GDPR guidance.
