AUTUMN 2 - After School Clubs - Request Form

Please choose the clubs that you would like your child to attend and complete the form below to register your interest. Be aware, clubs are phase/year group specific.

Deadline for registration: WEDNESDAY 30th OCTOBER as clubs will start wk beginning 4th NOVEMBER.

Places are limited and therefore if demand exceeds supply we will keep a waiting list and if a place becomes available we will contact you.

IMPORTANT: If your child gains a place they are committed to attending the club each week. If your child fails to attend for two weeks consecutively, without a valid reason, their place will be re-allocated to someone on the waiting list. Club attendance is also subject to behaviour
