Application for One on One Work
This application helps us determine if you are a good fit for our programs. Here's a brief summary of how it works and what to expect:

After you fill out this form, you'll be contacted by Amber's team within 72 hours and either given the option to book your initial session for the start of the next month or placed on the wait list.  

Initial sessions are $125 and not included in your program (though we have discounts on some of our programs to essentially refund this price if you move forward).  Because we only work with a small number of people each month, we only offer a few initial sessions at first.  However, the initial session is a no pressure meeting and is mostly a chance to meet Amber, make sure you will make a good team, and have her take a thorough health history from you.  You'll have some time after that session to confirm that you want to move forward.  

Because everyone has a chance to opt in or out after their initial session, sometimes spots do open up for the month, so if you're placed on the wait list, there's still a chance you could be selected to move forward.
