I, the undersigned, hereby apply to participate in the 3rd Pan-American Korean Para Games and pledge to comply with all rules and regulations set forth by the Korean American Para Sports Association and the Dallas Korean Para Sports Association. I further acknowledge and agree that I shall bear full responsibility for any and all physical injuries or mental issues that may arise during my participation in the competition. I hereby affirm that I will not raise or pursue any legal claims or actions of any kind against the Korean American Para Sports Association or the Dallas Korean Para Sports Association in connection with my participation in this event.
