SF Adult UWH Registration, Emergency Contact, & Releases
We require information about each participating athlete to have on file for emergencies and for our annual report, which is required to support our 501(c)(3) charitable status along with required information from certain donor organizations. Information shared will be kept confidential and only aggregate information will be shared outside of the Club.
 
This form must be filled out for each participating athlete once per year.
