Regular physical activity has many health benefits, both physical and mental, and being more active is very safe for most people.  However some people should check with their doctors before they start becoming much more physically active.

This health questionnaire must be completed in order to take part in any of my fitness sessions (outdoor group fitness, zoom classes and personal training), please complete your personal details and answer the seven health questions honestly.

If you are between the ages of 15 and 69, this screening form will tell you if you should check with your doctor
before you join me.  

If you are over 69 years of age, and you are not used to being very active, you must check with your doctor first and provide his written consent that he is happy for you to join my membership.
