Complete this form to register for the Winter 2022 Junior Gym Program. You will need to complete a separate form for each child that will be participating.

The K-3 program runs Mondays and Wednesdays from 3:30pm-4:30pm.

The 4-6 program runs Mondays and Wednesdays from 4:30-5:30pm.

The registration fee is $125.

PARTICIPATION IN ANY ACTIVITY WITHIN PEAK FITNESS IS AT THE SOLE DISCRETION AND JUDGEMENT OF THE MEMBER AT HIS OR HER OWN RISK

I assume full responsibility for death, injuries, catastrophic injuries or damages which may occur to me or my child in, on, or about the premises of the facility and do hereby fully and forever release and discharge PEAK FITNESS, the employees, and representatives from any and all suits, claims, damages, costs and expenses of every kind of conjunction with the use of PEAK FITNESS facilities and thereof equipment associated.

I acknowledge that the Junior Gym program hereunder includes participation in physical activities, including but not limited to various aerobic exercises, muscular endurance and or resistance training, flexibility and other physical measurements.

I acknowledge these physical activities may be strenuous and may involve inherent risk of physical injury.

As the parent/guardian, I agree to assume all risk and responsibility involved with my child's participation in these physical activities.
