Requests to change class delivery mode from IN PERSON or BLENDED to REMOTE or ONLINE requires approval.
Your Dean or Associate Dean(s) will be making the final decision.
Please communicate with your Associate Dean directly.  They will be given access to all information submitted via this form.

1) Complete 1 request per class (section).  Do not make requests for multiple courses or classes on the same request form unless the request is for Combined Sections.

2) Changes from IN PERSON/BLENDED to REMOTE/ONLINE requires approval.
