Baltimore Celtic COVID Game Day Screening
Baltimore Celtic is following specific guidelines and steps to reduce the risk of infection among players and staff in accordance with national and local restrictions, as well as the recommendations outlined by the CDC (Centers for Disease Control)

As a member of a youth soccer club community, I must answer the following screening questions prior to attending any sanctioned event that Baltimore Celtic is a participant.

All screening questions must be answered 2-3 hours prior to any Baltimore Celtic Game Day event, if you experience an issue with reporting, please contact your coaching staff for instructions.

Any participant answering Yes, to any of the below screening questions should contact their coaching staff immediately and NOT report to any Baltimore Celtic game day related event.

Has the participant experienced or is currently experiencing any any of the following symptoms: fever/chills, cough, sore throat, shortness of breath sudden loss of taste or smell, headache, diarrhea, fatigue, muscle aches, nausea/vomiting that began in the last 72 hours? *
Has the participant reported a a temperature greater than 100.4 degrees in the last 24 hours. *
Has the participant had a documented case of COVID-19 in the last 20 days. *
Has the player had any close contact or lives with an individual who has tested positive for COVID-19 or awaiting test results. *
Our family commits to following ALL Physical Distancing protocols while attending a Baltimore Celtic related game day event. *
