District 15 Chromebook Checkout Summer 2021 / Reservación Chromebook  Distrito 15 Verano de 2021
Please use this form to reserve a Chromebook for your student to use during McHenry District #15's Summer Learning. This form will need to be filled out for each student in your family that you would like to reserve a Chromebook. District summer tutoring will be available from July 5 through August 13.  Other resources will be available all summer long. You will receive an email or text with further instructions for picking up a Chromebook.

Utilice este formulario para reservar un Chromebook para que su estudiante lo use durante la tutoría de verano del Distrito 15 de McHenry.
