PFHS 2020-2021 Schedule Change Request Form (THIS FORM IS FOR STUDENTS WITH LAST NAMES BEGINNING WITH A - D ONLY)
Students: Use this form to request any changes you would like to make to your schedule. Please note that your scheduling NEEDS will be met based off of your graduation requirements, transcript, test scores, teacher and/or administrator recommendation. Graduation requirements will always take priority over elective courses. We will only honor valid academic requests.

Your requests will be reviewed in the order it was received.

Please fill this form out in its entirety. Your counselor may need to contact you if there are conflicts that we cannot resolve on our own. NOTE: This is a REQUEST ONLY, submitting this form does not guarantee your request will be approved.

Schedule change requests must be submitted by August 7, 2020. Please understand that requests submitted after this date cannot be guaranteed for review. Counselors and administrators will need time to review all requests and try to make changes. Counselors will address any changes that are necessary for graduation or promotion requirements.

All students will receive and are expected to use the copy of the current schedule listed in Powerschool. This schedule is the most updated and will include any changes made due to change requests or the balancing of classes. If the schedule is able to be changed, an updated schedule will be emailed. Counselors may use the contact information provided in this form to keep you updated on your schedule changes.

A Few Notes:
If you are requesting a FTCC, NCVPS, Teleconferencing, Early Release, or Early Graduation, additional paperwork will be needed. Also, you must meet the following requirements:

FTCC: 2.8 unweighted gpa or higher

NCVPS: 2.5 weighted gpa or higher. The class will be in addition to your regular scheduled classes. If the class you are interested in is offered face-to-face, you will not be allowed to take it online. Exceptions will be granted only for extenuating situations.

EARLY RELEASE: Must be a senior and still meet graduation requirements and need no more than six classes to graduate. If you are an ATHLETE, you must have three classes in the semester prior to your season. It is the responsibility of the student to make sure that they still meet athletic eligibility. The counselor will email you a copy of the contract after receiving your request. Your request will not be granted until this form is returned to the counselor.

MID-YEAR GRAD: Must be a senior who needs no more than four classes to graduate. The counselor will email you a copy of the contract after receiving your request. Your request will not be granted until this form is signed and returned to the counselor.

EARLY GRAD: Must be a junior who will have met all graduation requirements by the end of the school year. Your request will not be granted until this form is signed and returned to the counselor.

GRADPOINT: Must have failed a core course, missed no more than 30 days of the class and have course average higher than 40, OR received a WC19 as the final grade.
