Once you complete the online portion of the application, The Parental Consent Form and additional documents must be received in our office located at: CPFTA Attention: Nilda Duenas 42 W. Madison - Garden Level - GW311.10 Chicago, IL 60602 no later than 5:00 pm on Monday, May 6, 2019.
1. Complete the Online ApplicationAll applicants have until Friday, May 3, 2019 to complete the online portion of the CPFTA application.
2. Complete Your Consent FormAll applicants must complete the Parental Consent Form and turn it in signed and dated by School Principal. The form will be emailed to you after completing the online application (also attached to the last page of this online application).
3. Send In Your Consent Form and the following documents:• Two Letters of Recommendation (one from a school official and one from a community leader)• Current Physical (must be from within the last 12 months - NO SPORTS forms)• Copy of student’s Driver’s License or State Identification Card• Copy of High School Transcript for G.P.A. and Attendance verification• An application is INCOMPLETE until the signed Consent Form and documents are received in person (Attention: Nilda Duenas 42 W. Madison - Garden Level - GW311.10 Chicago, IL 60602), by mail (CPFTA Attention: Nilda Duenas 42 W. Madison - Garden Level - GW311.10 Chicago, IL 60602), scanned (email@example.com) or faxed 773-553-5600. The forms must be received in our office by May 6, 2019 no later than 5:00 pm. An application is INCOMPLETE until the signed Parental Consent Form and all other documents are received.
4. Chicago Police Department ProcessingApproximately 2-3 weeks after the Parental Consent Form and additional documents are received, the Chicago Police Department will mail you a packet which includes a "Personal History Questionnaire," a background investigation form, and an "Authorization for Release of Personal Information" form. These forms must be signed, completed and brought with you to processing day. Your processing date along with additional information will be in the packet.
