The above signed covenants not to sue and agrees never to institute any action of suit at law or in equity against the Carnegie Public libraries of Monte Vista and South Fork, Colorado, its agents or employees, or in any way aid in the institution or prosecution of any claim, demand, action or cause of action for damages, cost loss of services, or injury either to person or property, resulting or to result, known or unknown, and arising out of or from the undersigned’s use of the Carnegie Public Libraries Meeting Rooms.

I have read and understand the Carnegie Public Libraries Large Meeting Room Regulations and Policies.

Publicity for events held in the libraries meeting rooms shall not imply endorsement or sponsorship by the Carnegie Public Libraries and must include the following statement: "This event is not endorsed or sponsored by the Carnegie Public Libraries."

I understand that I am responsible for the set-up and take-down of the room. Please refer to the Carnegie Public Libraries Large Meeting Room Regulations and Policies.

I agree to pay a deposit of $25.00 to confirm the large meeting room reservation and understand that the $25.00 will be forfeited to help with cleaning or equipment replacement costs should the meeting room not be left in the condition I found it.
