Description, Date, Time, and Location of Field Trips:  
1. Senior Service Learning: through Theology class at assigned service sites in the Little Rock area for the school year
2. Mercy Day Service Project: September 22, 2023 at various service site locations in greater Little Rock  
3. Senior Class Retreat January 25, 2024 from 5:30 until 9:00 p.m. at MSM and January 26, 2024 from 8:00 a.m. until 1:00 p.m. at the 4-H Center  
4. Carmelite Monastery: April 18, 2024 from 9:30 a.m. until 12:15 p.m. on West 32nd near UALR
5. Baccalaureate practice:  May 17, 2024 from 1:00 p.m. until 2:00 p.m. at Christ the King Church in Little Rock
6. Graduation Practice: May 17, 2024 from 8:30 a.m. until 11:30 a.m. at Robinson Center in Little Rock
Method of Transportation:   Students will drive or ride with other seniors in personal vehicles.
Special Instructions:  Will be communicated to students before the events

I/We, give permission for my/our daughter to participate in above field trips under the conditions stated above.  I/We hereby release and hold harmless of neglect the school, any and all faculty, staff, and/or members of the administration of Mount St. Mary Academy for any harm to my/our daughter as a result of transportation to and from the event or as a result of the participation in the event itself.
