Check In Registration Form
This form is for single site registrations. To purchase the program for more than one site, you’ll need to complete the form multiple times OR contact us for manual bulk purchases.

The $500 starter fee allows for unlimited use of the Check In program for one year at a single implementation site (i.e. individual school, not a school district or individual workplace branch, not entire corporation). There is a $100 annual renewal fee, unless you opt out of continuation.

Once your application is reviewed and approved, you’ll be invoiced and shipped a starter kit that will include a program handbook, logos and graphics, 50 branded pens, table banner, electronic resources & more.
