We are researchers from the Occupational Ergonomics and Biomechanics Labs at Virginia Tech. We invite you to be part of a research study to enhance Parkinson's Disease rehabilitation. This research will help us develop a remote rehabilitation system for patients with Parkinson’s Disease. Please respond honestly and to the best of your ability.

Stage 0: no signs of disease

Stage 1: Unilateral involvement only

Stage 2: Bilateral involvement without impairment of balance

Stage 2.5: Mild bilateral involvement with recovery on retropulsion (pull) test

Stage 3: Mild to moderate bilateral involvement, some postural instability but physically independent

Stage 4: Severe disability, still able to walk and to stand unassisted

Stage 5: Wheelchair bound or bedridden unless aided
