Please note that this form is ONLY for students in the PRIMARY school, Kindergarten through 3rd grade. 

Please fill this form out if your child will be absent from school.  You may fill this form out at any time to excuse a current or previous absence. If you would like to report a future absence, please email psoffice@ichabodcrane.org. 

If you have multiple children, please fill out a separate form for each student in the primary school.
