Thank you for your interest in Team Up Mentoring’s Summer Camp! This pre-registration form helps us determine eligibility and secure your child’s spot in our program.

🔹 Camp Dates: June 9 - July 25 (Tues-Thurs)
🔹 Elementary Program: Rising K-5th graders
🔹 Teen Leadership Academy: Rising 6th-12th graders
🔹 Commitment: Campers must attend all 7 weeks
🔹 Cost & Payment: Fees must be paid in two installments (½ before camp, ½ midway through)

✔ Fill out one form per child.
✔ Carefully review and agree to all policies before submitting.
✔ Once submitted, you will receive a follow-up email with the official registration packet and payment details.
