Michigan DECA Merit Award Program Reporting Form for all levels
Deadlines:

December 1, 2020 - Bronze participants must be submitted in order to be recognized during the District Conference. (This is optional.)

February 1, 2021 - Bronze, Silver and Gold Merit Award participants must have their names submitted in order to receive a certificate, which will be mailed after the State Career Development Conference.

Submit completed projects to the Michigan DECA office by February 11, 2021 as per the event guidelines. Late projects will not be accepted.
I, as the chapter advisor, certify that the DECA member listed below has completed all projects of the Bronze, Silver and/or Gold levels of the Merit Award Program:
