Sex Education is mandated to be taught once in middle school and once in high school. California Healthy Youth Act (California Education Code Sections 51930-51939).

This FORM is to be completed by the legal Parent/Guardian.

This FORM serves as an official “Opt-Out” for SOME or ALL of the the comprehensive sexual educational and HIV prevention education and associated anonymous surveys as part of the Positive Prevention Plus curriculum to be provided to students at least once in middle school and once in high school.

This FORM may also be used to indicate if you have already satisfied the requirement or if you have elected an alternative method for addressing the content.
