The deadline for submitting all nominations is Friday 22 March 2019.
TO COMPLETE THIS FORM, YOU WILL NEED:
Nominee's full name, current year group, date of birth, gender, postcode, nearest town/city, whether they are a returning member or not
Instrument, information on any other ensembles/orchestras/groups they play in
Parent/guardian full name, phone number, email address, relationship to nominee

CONSENT FROM PARENT/GUARDIAN AND CHILD
The nominator must inform parents/guardians and young person that they are being nominated and gain their verbal consent to nominate them before doing so. They must also be fully informed of the process, detailed below. Finally, they must also ensure that the nominated young person can attend all 3 residential courses throughout the season:
4-7 August 2019 - Summer Course 201923-24 November 2019 - Winter Session 20197-9 April 2020 - Spring Course 2020
WHAT HAPPENS NEXT?
Nominations will close on 22 March and Orchestras for All will decide on places using the information given and criteria that is matched.
Young people will then be offered a PROVISIONAL place only. The nominating adult will be told of this provisional place for them to inform the young person. They will also be given a link to pass to parents/guardians to complete a consent and young person information form.
Fully completed parental consent and young person information form must be submitted by Monday 22 April 2019.
Once this is received, young people will then be offered a CONFIRMED place by Monday 29 April.
Failure to complete the parental consent and young person information form on time may result in the young person not getting a place on NOFA and it being offered to someone on the waiting list instead.
