I understand that if my child was NOT Baptised in St. Luke's Church Mulhuddart that I must submit a copy of my child's BAPTISM CERT. Baptism Certs are available from the church where your child was Baptised. These must be submitted to the Parish Office (in person, by post, scanned or photographed copy by email) no later than 31st December 2025.

I understand that if I have not submitted my child's Baptism Cert and Registration Form by 31st December 2025 my child may not be able to be included in the Confirmation Ceremony in 2026.

Name of Child's Confirmation Sponsor (if known)

Your child is required to have a Sponsor for their Confirmation. A Sponsor is someone over the age of 16 who is a Roman Catholic (who has made their First Communion and Confirmation). The Sponsor's role is to support your child as they grow in faith.

Your child is required to choose a Confirmation Name. Ideally this should be a Christian/Saint's Name and have some meaning or significance to your child.

I understand that the information contained in this form will only be used to contact me in connection with the preparation and celebration of my child's Confirmation. The information submitted in this form will be stored on an encrypted Excel spreadsheet on the Parish computer and will only be accessed by members of the Parish Team. The information submitted will be destroyed once Confirmation has taken place and has been recorded in the Parish Registers. The information in the Parish Registers will be retained permanently. If Baptism was in a different parish, information on Confirmation will be sent back to that parish for inclusion in their Baptism Register.
