unSCruz 2025: Cosmic Odyssey Parent/Guardian Release, Child Registration, and Agreement of Child Policy
Completion of this form is required for all children under the age of 18 attending unSCruz 2025: Cosmic Odyssey.
If you purchased a child ticket, you must fill out this form in order for your child to gain entry. Upon completion and submission of this form, you will receive an email confirmation. Print a copy of the confirmation email and bring it to the gate along with a printed photo of the child(ren) you are registering and the child ticket QR code (either printed or saved to your phone) for each child. Have the printed confirmation email, photo(s), and child ticket QR code(s) ready to present to the unSCruz person at the gate in order to be given a child wrist band. Your contact phone number during the event will be written on the wrist band.
For the purposes of this form, a child is any person below the age of 18.
By submitting this form, You, the adult(s) completing this form, who are the parent/guardian(s) of the child(ren) indicated, understand and agree to the following CHILD POLICY for the duration of the unSCruz event at the San Benito County Fairgrounds:
Parent/guardian(s) ARE ALWAYS RESPONSIBLE for their child(ren)s safety and actions.
It is the responsibility of parent/guardian(s) AT ALL TIMES to ensure their child(ren) are both reasonably cared for and behaving appropriately.
unSCruz and its agents and employees are released of all liability for the child(ren) listed below.
Any damage caused by the child(ren) listed below while attending unSCruz IS THE RESPONSIBILITY AND LIABILITY OF THE UNDERSIGNED and may lead to event eviction of ALL undersigned parties and compensation paid to the owner of the damaged property.
If the Rangers observe that the child(ren) is/are not being properly cared for at the event, or that a child(ren)'s behavior is/are causing significant problems, they will work out an appropriate response, beginning with a conversation with the parent/guardian(s), up to and including expulsion from the event.
Parent/guardian(s) must be able to immediately locate their child(ren) at all times.
All children at unSCruz must also always be able to locate their parent/guardian(s) at all times.
All children must have a parent/guardian's phone number written on their wristband for the entirety of the event. unSCruz will provide the wristband for this purpose.
The parent/guardian(s) have the responsibility to ensure the phone referenced on the child(ren)'s wristband is reachable and has a signal.
IF AT ANY TIME a parent/guardian loses the ability to locate their child(ren), they must IMMEDIATELY find the Rangers and report their child(ren) as lost. The Rangers will reunite them.
Consent is given by you, the adult(s) completing this form, who are the parent/guardian(s) of the child(ren) indicated, and the child(ren) listed below to receive medical treatment, which may be deemed advisable in the event of injury, accident or illness during this activity or event. This release, indemnification, and waiver shall be construed broadly to provide a release, indemnification, and waiver to the maximum extent permissible under applicable law.
Rainbow Trike Track and Fire Art
All children at the Trike Track, or within 10 feet of any fire art installation, MUST BE ACCOMPANIED by a designated responsible adult. This means NO UNSUPERVISED CHILDREN UNDER 18 ARE ALLOWED
at the Rainbow Trike Track or within 10 feet of any Fire Art installations AT ANY TIME.
You will receive an email confirmation upon submission of this form. Please print it out and bring it with you to the event along with a PHOTO and a valid child ticket QR code for EACH CHILD you are registering. This CONFIDENTIAL information will be securely held with High Rock Security for the duration of the event and DESTROYED post-event.
