Please Fill Out the Following form- only complete forms will be accepted. A follow up email will be sent once received with payment instructions. All VIP tables are non-refundable. A 50% deposit is due upon receipt of invoice. Your reservation has not been confirmed until you receive a confirmation email from SRJT and payment has been received. Remaining 50% will be due February 1st, 2026. Card on file will automatically be charged if payment is not updated prior to 2/1/26. If paying with a check, the check must be received by 2/1/26.
