This form should be completed annually by parents and students wishing to participate in TFCA Athletics. It does NOT have to be completed separately for each sport, simply filled out once per school year.

The Athletic Director and Finance Office must receive this form, along with a current KHSAA Physical Examination form as well as the athletic fee in order for a student to participate.
The physical must be on file with the office prior to a student participating in their first practice or a parent must remain at the practice until a physical is turned in to the AD.
