FLCC Contact Information Form
The purpose of this form is:
  • To provide your emergency contact phone number and email address to the FLCC Board.
  • To create the FLCC Community Directory.

The directory is available only to community members who complete and submit this form. It includes your name, bungalow address, and any contact information you'd like to share.

Please have each of your bungalow's shareholders submit a separate form.  

Note: After submitting this form, you'll receive two emails. Save them both.  One has the link to the directory. The other has a link for updating your information in the future.

Mobile Phone *
Mobile phone is required for the Board's emergency contact list. If you don't have a mobile phone, enter here the best number for the Board to reach you and tell us what kind of number it is in the Comments at the end of this form.
Bungalow Landline
If you don't have a bungalow landline, leave this field blank.
Winter Landline
If you don't have a winter landline, leave this field blank.
Opt Out
By default, all contact info is included in the FLCC Community Directory.
To exclude specific contact info from the directory, check the corresponding boxes.
Extended Community
To foster greater community connection, you can add names of people who frequent your bungalow and their relationship to you. Otherwise, leave blank.
