The purpose of this form is:
  • To provide your emergency contact phone number and email address. (FLCC Board use only)
  • To let the us know if you have/want access to the For Cooperators section of the forestlakecc.com website, if your name is on the stock certificate, and how long you've been a part of FLCC. (FLCC Board use only)
  • To create the FLCC Community Directory.

The directory is available only to community members who complete and submit this form. It includes your name, bungalow address, and any contact information you'd like to share.

Please have each of your bungalow's shareholders submit a separate form.  

Note: After submitting this form, you'll receive two emails. Save them both.  One has the link to the directory. The other has a link for updating your information in the future.
