Grant Request Form

All non-profit organizations serving children are eligible to apply!

Our goal is to find donors to support your work with students. We will NOT ask you to spend hours filling out an online application! Simply fill out a request form for your project and materials and some basic information.

Please be sure to gain the permission of your principal or director before you begin. Also, share the Math & Movement website with your colleagues to determine how many of them are interested in being involved . You will also be asked if you are willing to evaluate your students with pre- and post-testing and some questions about the demographics of your school. The check for your project will come directly to your organization. Your project will be listed on the website for donors to consider.

After you have received the donation, we request that you honor the donor by writing and having your students write a thank you note! Given that release forms are available, we ask that you send us pictures and videos of students engaging in the activities and the copies of the thank you notes. We will include this information on the website. Please also send us the copies of pre- and post-tests. We will compile the results into a final report and submit to the donor. The results will also be included in other data collected by teachers for a federal innovations grant.
