Please provide as much information as possible regarding the incident, suspicious activity, or concern. Include a detailed and objective summary of what occurred, including who was involved, what happened, when and where it occurred, and any other relevant details.

Helpful information may include:
  • Names of involved individuals, suspects, witnesses, or persons of interest
  • Suspect/person of concerns information, including name, address, and identifying characteristics such as age, sex, race, height, weight, hair color, eye color, tattoos, scars, or other distinguishing features
  • Vehicle information, including make, model, color, year, license plate number(s), and other identifying details
  • Known friends, associates, or individuals connected to the suspect(s)
  • Relevant dates, times, locations, or patterns of behavior
