Chapter advisers, please complete this application to nominate your chapter for the Membership Campaign Award. which will be awarded during the 2nd General Session at the 2021 State Leadership Conference.

- Nominations must be submitted on or before March 1, 2021, to be considered
- One chapter will be chosen
