Thank you for purchasing a ticket, please complete this form to finalise your child's participation in the Makers Play Lab Summer Holidays Program 2021. Please note we can only process 2021 vouchers from now on.
Your child's attendance will be confirmed once the voucher has been processed. It takes 7 days to process the voucher with Service NSW. We will notify you if there is an issue processing the voucher. You must complete a seperate form for each child you register.
