W2 Electronic Copy Request Form
All W2 forms for the most recent tax year will be mailed out on or before 01/17/2024 via USPS postal mail. The organization also has the ability to send out W2s electronically for those that request and consent to the W2 electronic copy request. These are sent out via email only. 

Requests are processed every Thursday by the payroll department. If you do not receive your W2 electronic copy within 7 days of your request, please notify the office. Individuals must fill out this request form to receive their W2 electronically.
