THIS REQUEST FORM IS FOR THE USE OF CURRENTLY ENROLLED SHHS JROTC CADETS.

This form serves as a acknowledgment of any SHHS JROTC cadet's request to be REENROLLED in the JROTC program for the next school year.

NOTE: The completion of this request form DOES NOT automatically enroll any student into the JROTC program. An Army Instructor (AI) from the SHHS JROTC program will review submitted information before enrollment is officially requested. 

Again, enrolled Cadets incur ZERO obligation to join the military as a result of their enrollment in JROTC.
