CDA Funding Application Form
An application for CDA funding shall be accepted to cover the cost of the initial online assessment fee, the renewal online application fee, and the CDA Competency Standards Book.

Applicant must be a registered family child care provider or a licensed child care center staff in the State of Maryland.
