Manual Correction/Deletion Request Submitted
The Special Manual Record Correction Request section in Chapter 5 was updated with v1.19.1 of the MDS 3.0 RAI Guidance Manual effective October 1, 2024, adding new rules for how to remove an MDS record from iQIES when the submitted record was “not for OBRA or Medicare Part A purposes.” The new rules address erroneous standalone assessments, as well as erroneous assessments that are combined with OBRA-required assessments.

This form is to be completed in its entirety and submitted to the State RAI Coordinator.
