Class Change/Substitute TA Notification Form
If you need to miss a class, please make the arrangements to find a substitute teacher before filling this form.

Fill this form with at least one week advance to notify CYMP's HR Dept (AKA Alvaro) that you will be missing a class and who will substitute you for it. If no substitute TAs are available, please get in touch with Liz Ramirez, Program Coordinator, to try to find a different schedule for the class, or for a substitute TA to cover you on a different schedule. If no option is found, select the "cancelled class" option.

Your substitute TA should be part of the current staff, and you should make the arrangements to be sure the substitute TA is able to cover for you.

If you will miss more than one class, please complete one form per each class you will miss, (fill, submit, and fill a new form).
