Note: ANY CHANGES TO NATIONAL FORM 1 RECORDS, PRIOR TO DATE OF FINAL SUBMISSION FOR APPROVAL IN THE NATIONAL SYSTEM WILL REQUIRE A NEW CERTIFICATION FORM FOR THE PROGRAM & TEAM.
COMPLETED FORM MUST BE SIGNED BY THE ZONE CHAIRMAN, verifying they have reviewed one of the following for each player
- Approved National Player ID Card
- Certified Original Birth Certificate
- US issued Passport
- Valid CT Federal ID Driver's License or
- Original foreign birth certificate (with raised seal)
and letter of translation if required
- Each player meets the age requirements as set forth by the American Legion National Rule book (Rule 2)
Penalties for falsifying this form will be suspension of the coach and all games played will be forfeited.
