All Class Proposals will require that you provide a Course Outline, Course Objectives, and a Course Outline. These additional documents can be found on the Summer School website. If you need to purchase supplies, a Budget Requisition form will be emailed to you with instructions. If your class will have any field trips, a SPSD Field Trip Request will be emailed to you, also. ALLERGIES - List any projects or class activities that may involve food, snacks or drinks. The District's Wellness policy (8510) is followed at Summer School. Please take the time to read it. No food or candy, sugar free or not, is allowed to be distributed to students unless approved by the Summer School Principal. *
Laura Cody, RN, District Nurse, must be notified of ANY food or drink served to students. She will check your roster for food allergies and inform you. Many children may have food allergies. We take this very seriously.

What type of classroom is needed for your class? We will do our best to find a space that works best. *
We no longer are able to provide TV's with VCR's or DVD players. Smartboards are not available in all classrooms. Generally, Chromebooks will be made available on a check out basis, if they are available.

FIELD TRIPS - WALKING OR BY BUS (Due to Maria by late Jan 2018) *
If you are planning any class outings that leave campus with students by bus or walking, list dates & destinations. If you do not know all the specifics just yet, tell us what you do know. We understand that there is planning to be done. We do need to know of your intentions and will follow up by sending you the District's FT forms to complete.
