2019 - 2020 Early Dismissal Approval Form for Seniors

Dear Parent/Guardian and Student,

Seniors are permitted to leave High School East after their last scheduled class.  In order to so, the form below must be completed and digitally signed by a parent or guardian, and the student.

When leaving the school grounds, students MUST show the security guard their "Early Dismissal Pass."  Failure to follow early dismissal rules could result in the loss of parking AND/OR early dismissal privileges.

Please read and complete the form below.  When processed, passes can be picked up in Room 742.  If you have multiple children in the Senior class, please fill out a form for each child.  If you have any questions, please call my office at 592-3114.

Early Dismissal passes can be picked up in room 742 during the following dates and times after having submitted this form.

-Tuesday, August 20 (8-11am)
-Wednesday, August 21 (8-11am)
-Thursday, August 22 (8-11am)
-Tuesday, August 27 (8-11am)
-Wednesday, August 28 (8-11am)
-Thursday, August 29 (8-11am)
