This form is just for authorization to use Honorlock in a combined online/hybrid/on-ground course shell.
If you would like assistance in setting up and managing the deployment of your exams within Honorlock, please fill out the 'Exam Request Form' (
https://app.smartsheet.com/b/form/ec7e48f6ff074216978887a563efade3
) at least one week before your exam is due to launch. Requests submitted less than 3 business days before the exam may not be able to be completed.
