Chromebook Damage Report
Whether your Chromebook is not functioning properly, has suffered some damage, or is missing, it is the student's responsibility to let a school official know as soon as possible. Never try to repair your Chromebook yourself. Students/Parents must submit a Chromebook Damage Report directly to the IT Department in order for devices to be assessed and repaired. The IT department and Administration will determine whether the damage is covered under the coverage plan or if additional resolutions/actions are needed.

Cosmetic Damage: Select this option if the Chromebook assigned to you has been superficially damaged
Detached Keys: Select this option if one or more keys is detached from the Chromebook assigned to you
Damaged Screen: Select this option if the screen has been damaged or is otherwise not functioning on the Chromebook assigned to you
Software issue: Select this option if a software error has occurred that renders the Chromebook assigned to you to inoperable
Hardware Issue: Select this option if the Chromebook assigned to you is experiencing a hardware issue and is rendered inoperable
Missing Charger: Select this option if you have misplaced the charger associated with your assigned Chromebook
Missing Chromebook: Select this option if you have misplaced your assigned Chromebook
Description of Damage
Describe in detail the extent of the damage and how that damage occurred
A copy of your responses will be emailed to the address you provided.
