Thursday, November 8 - Saturday, November 10, 2018 on the campus of the University of Michigan - Ann Arbor

The deadline for completion of this questionnaire is 12:00am ET on Thursday, October 11, 2018.

This questionnaire contains FOUR sections. In the FIRST section you will be asked to complete basic contact information such as name, address, email, etc.  Section TWO contains a variety of questions related to your academic background; it may be helpful to have a transcript on hand to help you complete this section. Section THREE will ask for preferences about this event.  In this section you will also need to provide the name and email address for one faculty reference from your home institution. The FINAL section asks for some demographic and background information.

Responses to this questionnaire will be reviewed on a rolling basis. Invitations to our Preview Weekend will be issued by Friday, October 19, 2018.
