Miracle League of Plymouth Group Request Form
Please fill out this form if you would like to bring a group of volunteers to buddy with our players during a game. The volunteer software we use only allows one individual to register at a time, so if you want to ensure your spots are reserved, please provide the following information so we can put a placeholder in for you.

You will be notified on whether or not your request was approved within a week. 

Games taking place on October 19 & 20 will be Halloween Themed Games! We ask volunteers on those days to come to the field in costume to join in on the fun! 

We will notify you at least 2 hours before gametime if we are cancelling due to weather. We post news on our social media pages first then we will send out an email.

What is the name of your group/team/organization? *

This is the person who will send out reminders to their group and be at the field on gameday helping our staff with check in. If someone else will be at the field on gameday, please include that in the comment section below.

How many individuals do you plan on bringing? *
This number indicates the people going on the field to work with our players. Not parents/guardians/family members coming to watch. 
GAME CHOICES: What are your preferred date(s) and time? List the date, time, and teams playing. *
This will be approved on a first come- first serve basis. 
Do you plan on bringing a photographer? *
If so, the individual will need to register on our volunteer software as well. Our players do sign off to having their pictures taken. We only ask you provide us a copy of your photos to include in our Facebook Albums. A MLP Staff member will work with you the day of to coordinate that.
What are your groups social media handles? We want to show our appreciation for every group who comes out to help and welcome you to tag our organization if you make any posts. Awareness helps everyone! *
