Safer Sex Supplies Order Form  
We are located at 333 East Campus Mall, Suite 3143. We are at the end of the hall on the 3rd floor. Your orders will be placed below our safer sex supply table which you are also welcome to take supplies from! Please share this form with any UW students who may be interested! Reproductive Health Items
*Sex Out Loud would like to note that some of these items were DONATED to us by the EC Access Network (Advocates for Youth Program) as well as directly from Plan B's Donation Program. We ask that you only order these items in case of an emergency*

If anyone needs items urgently or would like to stock up, please consult the following:

- UHS offers FREE pregnancy tests on the 6th floor of 333 East Campus Mall
- Badger Markets on campus offer emergency contraception (the morning after pill) for a LOW COST ($13) 
- Planned Parenthood clinics in Madison provide emergency contraceptive for FREE or at a LOW COST as well
-Community Pharmacy offers free Plan B at their location in Madison

*Orders of these are limited to TWICE per month as we have a limited supply. Please try to follow this rule as we would like everyone to have a chance to order them as needed.
