Lower School Planned Absence Notification
The purpose of this form is to notify the Lower School of an upcoming planned absence.
Work will be provided when the student returns to school.
Please keep in mind that when a student misses class, he/she is also missing classroom instruction and discussion that cannot be made up by normal homework assignments or within the regular school day. If you have multiple children, please fill out a form for each child.
Note: This form is not required for traditional medical/dental appointments. Students are allowed to miss up to six days of school per semester. There are no distinctions between excused or unexcused absences.
