DOWNERS GROVE BICYCLE CLUB RELEASE OF LIABILITY - READ BEFORE SIGNING
In consideration of being allowed to participate in any way in the Downers Grove Bicycle Club (hereafter: 'Club') program, its related events and activities, I, the undersigned, acknowledge, appreciate, and agree that:

NEW - By signing this waiver,
I hereby grant permission to the Downers Grove Bicycle Club (the "Club") and its representatives, agents, and employees to take photographs, video recordings, and/or other media (collectively, "Media") of me and my property, including bicycles and equipment, in connection with my participation in any Club-sponsored ride, social event, or activity. I authorize the Club to use and publish this Media in print and/or electronically. I agree that the Club may use such Media for any lawful purpose, including but not limited to, publicity, illustration, advertising, and web content. I understand that my name, likeness, and image may be used in connection with the Media, and I waive any right to inspect or approve the finished product, including written copy, wherein my likeness appears. I understand that all Media taken are and shall remain the sole property of the Club, and I waive any right to royalties or other compensation arising from or related to the use of the Media.
I understand that I may request the removal of any media in which I am identifiable from public platforms by contacting the Club at info@dgbikeclub.org. The Club will make reasonable efforts to honor such requests in a timely manner. I release, discharge, and agree to hold harmless the Club, its directors, officers, employees, and agents from any and all claims, demands, or causes of action which I may have now or in the future for libel, defamation, invasion of privacy, or violation of any right of publicity, arising out of or in connection with the authorized use of the Media.
2. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
3. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
4. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such hazard to the attention of the Club immediately; and,
5. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMINIFY, AND HOLD HARLMESS the Downers Grove Bicycle Club, its officers, officials, agents and/or employees, other event participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners, lessors and lessees of premises used for the activity ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property associated with my presence or participation, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTANDING ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY THREAT, DURESS, PROMISE, OR INDUCEMENT.
